Pre-IVF Screening and Consent Requirements
Informed Consent
Familiarize yourself with the Risks Associated with IVF. Informed consent is needed so that patients understand the risks involved in achieving an IVF pregnancy. Consent forms must be completed with acceptance of a treatment cycle. Consents will be sent out using our consenting platform EngagedMD when treatment is offered. You will be able to DocuSign and your forms will be electronically returned to RFP.
Screening
In order to maximize pregnancy rates it is important to perform testing on both partners to ensure that the sperm, ovulatory and uterine environments are optimized. All of the test results are necessary and should be current (within one year of treatment).
Female Screening Requirements
- Blood Tests:
Day 3 (of un-medicated cycle) FSH and Estradiol levels
Rubella titre
Blood type
AMH
HIV I and II
HTLV I and II
Hepatitis B surface antigen
Hepatitis C
Syphilis - Pelvic ultrasound to rule out ovarian cysts, fibroids, polyps and hydrosalpinges (fluid filled tubes)
- Hysterosalpingogram (HSG), sonohysterogram (SHG) or hysteroscopy to evaluate the uterine cavity and tubal patency.
Male Screening Requirements:
- Semen Analysis (a sample produced at our lab is preferred) with antisperm antibody testing
- Blood Tests:
HIV I and II
HTLV I and II
Hepatitis B surface antigen
Hepatitis C
Syphilis - Blood tests for cystic fibrosis and karyotype may be required if the semen analysis shows low numbers and ICSI is required.
Note: Additional tests and counseling are required for the use of donor sperm.
