What are my options when I no longer wish to continue storing my embryos, eggs or sperm?
Patients can choose the following options:
Destroyed – Log onto the Prelude Cryopreservation website of the Prelude Network and enter your Prelude Cryopreservation account credentials. Under Manage Services, click on Discontinue Service. Please select the tissue that you would like to Discontinue Service for Eggs, Embryos or Sperm and follow the prompts. This confirms and authorizes the lab to discard ALL selected tissues associated with All Freeze or Receive Dates and that you understand that your decision will result in your inability to use these cryopreserved specimens for future reproductive cycles. Your cryopreserved tissue will be destroyed in the IVF lab after receipt of your consent. You will be notified by mail after the destruction. It is important to inform the clinic of any change in address or contact information. Shipped to another Clinic – contact the clinic if you wish to arrange transportation of your embryos or reproductive tissue to another clinic.
Donated to the RFP Embryo Donation Program – contact the clinic if you wish to donate your embryos or eggs to another infertile patient or couple. Unfortunately RFP does not have a program for sperm donation. You will be contacted by mail with information and consents describing your interest to participate in the Embryo Donation program. Donors are required to complete a detailed family history form, undergo blood tests to screen for infectious diseases and have a consultation with one of our counsellors prior to donation of their embryos or eggs. The consultation with a counsellor is usually done by telephone. A current mailing address needs to be maintained with the clinic. The following criteria are requirements for patients wishing to donate their embryos or eggs:
1. female partner must be 37 years or younger at the time of egg retrieval
2. fertilization of the eggs must not have been done with intracytoplasmic sperm insertion (ICSI) (unless it was done for antisperm antibodies, previous vasectomy or failed fertilization).
