You are a woman UNDER 35 who is not yet pregnant after trying for one (1) year
You are a woman OVER 35 who is not yet pregnant after trying for six (6) months
You have experienced repeated pregnancy loss
You are a women or a man who has been diagnosed with cancer and you want to preserve your fertility
You are a single woman and want to have child
You are a woman or a man in a same sex relationship and you want to have a child
You are a woman between 30 and 35 who wants to delay childbearing for personal reasons
You identify as a transman or a transwoman and require assistance with either fertility or fertility preservation
Previous history of vasectomy
You are concerned about passing on an heritable genetic disease to your offspring

To book an appointment with our fertility specialists, please have your Canadian physician send us a letter of referral. The usual wait for consultation is approximately four to five weeks, depending on the urgency of the referral.
Referral Information for Patients
Please make sure to initiate a referral for yourself and anyone who may be involved in your family building journey. Wait times for consultation vary. We will contact both you and your referring provider within 14 days of the receipt of your referral to confirm we have received it.
Referral Information for Physicians
At present patients are accepted for treatment by Canadian physician referral only. Letters of referral may be faxed to (403) 284-9293 . For your convenience you may fill out our paper Referral Form. The usual wait for consultation is approximately four to five weeks depending on the urgency of the referral. Unfortunately, due to medical/legal coverage only Canadian residents can be accepted for treatment at the Regional Fertility Program.
The Regional Fertility Program processes referrals according to CPSA Guidelines. A referral is required for each patient; please refer both the patient and their partner (if applicable), or guide them to ask their healthcare provider to do so for faster processing. Our coordinators will reply by fax within 14 days and provide referral status updates for your requests per guidelines.
