Pre-IVF Screening and Consent Requirements

Informed Consent

Familiarize yourself with the Risks Associated with IVF. Informed consent is needed so that patients understand the risks involved in achieving an IVF pregnancy. In addition, all consent forms required for an IVF cycle will need to be returned to the clinic fully completed before you are offered treatment. If a witness is needed to sign your consent forms, you / (and your partner) can come into the clinic for assistance from the reception staff. You / Both need to be present with valid government issued picture ID. As of September 1, 2020 there will be no exceptions to this rule.
